The three tiers of govt must work in tandem by strengthening all health facilities to bring scrub typhus under control
The threat of scrub typhus is real. While people are still struggling to recover from dengue outbreak, the Kathmandu Valley is now witnessing rapid spread of scrub typhus, also known as bush typhus, which is caused by parasites found in rats and mice. The number of patients suffering from scrub typhus has more than doubled this year compared to last year. According to Epidemiology and Disease Control Division (EDCD) of the Department of Health Services, the Valley reported 15 cases of scrub typhus in 2018, while the number of scrub typhus patients has more than doubled to 40 this year. Many people may have been infected with the disease, but they are not diagnosed properly as the symptoms of scrub typhus are similar to that of dengue. The most common symptoms of bush typhus are fever, headache, body aches and rashes, similar to dengue. But scrub typhus can be more fatal than dengue if one does not take appropriate treatment on time. The EDCD has said as many as 695 people have tested positive for scrub typhus in 61 districts between July 17 and September 23. In 2018, the number of people suffering from the disease stood at 1,098 nationwide. The possibility of the people contacting the disease cannot be ruled out as harvesting season has yet to start. People who work in the farmland may contact with rats.
Scrub typhus is an infectious illness caused by a parasite – chigger – which is found in rats and mice. The mites in their larval stage contact the disease by biting rodents. Infected chiggers are mostly found during the wet season in forest clearings, riverbanks and grassy fields. The Tarai region has the suitable environment for parasites causing scrub typhus. Therefore, people should avoid contacting with rats and mice at the time of harvesting paddy. According to studies, up to 30 to 70 per cent of those severely affected by the disease die globally due to lack of timely treatment. The bacterium that causes the disease severely affects bloods vessels leading to failure of vital organs such as kidneys and liver. This year, the disease has also been seen in high altitude areas due to the movement of people and climate change.
Considering the severity of the disease, doctors have advised the people to contact with the nearest health facilities in case such symptoms appear because mortality rate from scrub typhus is higher than from dengue. People can die if they do not receive proper treatment on time. So far, Dhading (74), Dadeldhura (61), Palpa (59), Rupandehi (46), Kailali (44) and Chitwan (39) are the districts most affected by scrub typhus, which is usually seen in August and September. This is the time when bushes grow rapidly by providing a fertile ground for the rodents to multiply. In order to bring the disease under control, the federal, provincial and local governments must work in tandem by strengthening the local health facilities and provincial hospitals along with the provision of modern diagnostic kits and adequate supply of medicine. An awareness campaign should also be launched in the affected districts, especially among those working in the fields and the forests. It is the duty of the three tiers of government to ensure right to health services to the people.
End extortion spree
On the eve of the Dashain festival, the Netra Bikram Chand-led Communist Party of Nepal is reported to be on an extortion spree in Bajura in far-west Nepal. It has targeted government offices, traders, people’s representatives as well as teachers and health personnel. According to the victims, the group has set the amount of donations to be handed over, which could be as much as a month’s salary. The party cadres know very well that everyone will be getting a Dashain bonus sometime next week – which amounts to a month’s salary – and it should have no difficulty collecting it as people easily give in to their threats and intimidation.
The ongoing extortion is not an isolated incident. They’ve used manhandling and bomb blasts to intimidate both people and organisations even in the capital. The state is bound to provide, if nothing else, at least security to its people. As millions of people will be travelling to their homes carrying some cash and expensive gifts for the festival, they could be easy targets on the highways. So the government is advised to bring the nefarious activities of the Chand-led party – and other groups – under control sooner than later.

Dengue is a major public health problem in the tropical and sub-tropical regions, including Nepal. It is a mosquito-transmitted disease, characterised by fever, headache, muscle, joint pains, rash, nausea and vomiting.

The first case of the dengue virus in Nepal was observed in 2004. Since then major outbreaks have taken place in 2010, 2013 and 2016. This year, more than 5,000 cases of infection along with several casualties have already been reported from different districts. Our previous research and publications show that there is a dengue outbreak every three years, and the 2019 outbreak is as per our prediction.
According to this prediction, there is again going to be a major dengue outbreak in 2022. The major epidemics to date have demonstrated their capacity to be explosive and challenging to currently available disease control measures.
Dengue has four serologically distinguishable strains, DEN-1, DEN2, DEN-3 and DEN-4, which are capable of causing dengue fever (DF). Although infection with one serotype confers lifelong protection against that serotype, it does not necessarily protect one against a secondary infection with a heterologous serotype. This is because of the formation of memory cells of the previously attacked serotype of the virus. It is interesting to note that these serotypes also change from year to year with the emergence and re-emergence in different time periods. The first case of dengue infection that started with serotype 1, 2 and 3 in 2006, continued with serotype 1 in 2010, serotype 2 in 2013 and 1 in 2016.
The female Aedesaegypti mosquito, the transmitter, commonly lays eggs on the inner walls of artificial containers that are filled with water. The adult mosquito then can fly from one area to another. So our control strategies should be focussed on removing mosquito dwelling places, such as old tyres, cans or flower pots that collect rain. The dengue virus is spread through a human-to-mosquito-to-human cycle. Humans are the main amplifying host of the virus. This means that people, rather than mosquitoes, rapidly move the virus within and between communities. So there is a high chance of transmission in overly crowded regions like Kathmandu, where the rainfall and climatic factors are also favourable. A single mosquito can infect many people at a time in such conditions.
Currently, there are no effective vaccines or antiviral drugs against the virus. This problem thus needs to be addressed as a matter of urgency as failure to develop effective control strategies will inevitably increase the number of infected humans and expansion of infected territories regardless of the climate and altitudinal factors. This year’s outbreak saw its territory expand to more than 44 districts. Studies over the past few years have also shown that the months of infection have also expanded the timeframe from July to October and even December at times. This means that we still have more months to struggle with.
Dengue can be diagnosed by isolating the virus, by serological tests, or by molecular methods. The dengue viruses RNA and NS1 are detectable during the first week of illness. Anti-DENV IgM is detectable starting approximately five days after the onset of the illness. Diagnosis of acute (on-going) or recent dengue infection can be established by testing serum samples during the first five days of the symptoms or early convalescent phase (more than five days of symptoms). The correct diagnosis of DENV in the preliminary stages is very important to prevent the patients from more severe complications. Rapid diagnostic kits should be made available to all infected areas so that early diagnosis can be made. Cost effective diagnosis or cost-free treatment should be made available to the infected people.
“Diseases and natural calamities do not differentiate between rich and poor.” Keeping this in mind, we must prepare to protect ourselves from the attack of diseases. There should be combined efforts of the government bodies down to the local level to implement the control strategies. There is a pressing need to undertake effective vector surveillance studies, backed by well-equipped diagnostic virology laboratories. The infection is nearly out of control this year, and in this condition, the government can utilise the graduates and health professionals to create awareness about the control methods.
Capacity-building has been neglected at all levels of dengue prevention and control. Effective training must be scaled up to the national level. Social scientists and communication specialists, public health entomologists, vector control personnel, epidemiologists, diagnostic laboratory staff, and healthcare personnel play essential roles and need to work together. Prevention is always better than cure.
As there is no specific medicine for dengue infection, personal protection strategies are the best way to stay safe. Use of mosquito repellents, staying indoors in the evening, wearing long-sleeved shirts and long pants and timely visit to the hospital and avoiding unwise use of antibiotics are the ways to protect oneself. Furthermore, community-based control strategies along with the participation of the media can play a pivotal role in creating awareness in the people.

Leader of the Communist Party of China visiting Nepal Song Tao today called on President Bidhya Devi Bhandari and Nepal Communist Party Co-chairperson Pushpa Kamal Dahal.
During his meeting with Bhandari at the latter’s official residence, Song said China was always in favour of Nepal’s development.
Song, who heads the CPC’s International Liaison Department, said to achieve development, political parties in Nepal should enhance their capacity and be strong, according to the president’s advisor Sushil Pyakurel. Song said the MoU between the CPC and the NCP was in line with the idea of strengthening the NCP.
In her reply, Bhandari said Nepal was not a people’s republic, but federal democratic republic, and that any party could come to power through the election process.
That’s why, she said, the CPC should have harmonious relations with all Nepali parties because Nepal-China relations were above party-to-party relations, according to Pyakurel.
The meeting between Dahal and Song at a hotel in Kathmandu mostly dwelt on activities of the communist parties of the two countries.
Dahal thanked the CPC for extending cooperation to Nepal and Nepali people. He stressed the need for further strengthening relation between the two parties through continuous experience sharing. “I expect additional support from the Chinese government and the CPC in Nepal’s development endeavours,” Dahal’s secretariat quoted him as saying, during the metering.
Song stressed on the need for building a common platform for experience sharing between the two parties. Stating that ideology was one of the biggest assets of a communist party, Song said both the parties should work in further strengthening their ideologies.
“Parties can win people’s heart only through development,” said Song was quoted as saying by Dahal’s secretariat during the meeting.
Yesterday, Song had called on Prime Minister KP Sharma Oli at the latter’s official residence, in Baluwatar. According to Oli’s Press Advisor Kundan Aryal, Song said his Nepal visit was also part of preparations for upcoming Nepal visit of Chinese President Xi Jinping. Prime Minister Oli said he hoped Xi’s Nepal visit would be fruitful. Going against the government law, unregistered private firms and individuals have been collecting parking fees in a rampant manner from vehicle owners. In fact, such firms and individuals can be seen in and around government offices that witness a large flow of service-seekers like transport offices and land revenue offices.
These aforementioned firms and individuals have been providing fake parking slips and collecting fees, which at times can be exorbitant, from vehicle owners. They do not have any agreement with any government agency for the service and the government meanwhile is also losing out on taxes.
For instance, Anuradha Karna, a two-wheeler owner, had recently gone to the transport office in Ekantakuna of Lalitpur to pay her annual vehicle renewal fee. She says that she had parked her two-wheeler on public land near the office premises and was approached by a person seeking Rs 50 as parking fee. In another similar incident, a man was asked to fork out Rs 200 for parking his car in the area for two hours.
When apprised about the parking fee incidents, Prem Prasad Bhattarai, chief administrative officer at Lalitpur Metropolitian City, said that this was illegal. “We have not noticed this fraud but if this is true, then we will definitely take action against such firms and individuals,” he said, adding that the office will notify the police about it and ask them to take necessary action.
Bhattarai further added that they have not authorised any firm or individual to collect such fees and would charge them under fraud case.
This problem is not limited to Ekantakuna as one can notice vehicle owners having to pay parking fees even in the transport office in Sano Bharyang of Kathmandu.
Meanwhile, Ishwor Man Dangol, spokesperson for Kathmandu Metropolitian City, informed that they have not authorised any firm or individual to collect parking fees in the above mentioned areas. “The only areas that we have assigned authority to any firm to collect parking fee is in the core places of the city like Dharmapath, Khichapokhari, Sundhara, Shukrapath, Pyukha, New Road Pipal Bot, Shankardev Campus, Tridevi Marg (Thamel), Durbar Marg and New Baneshwor.”
“We have heard that some people are collecting parking fees illegally in certain places and we will soon initiate legal action against them,” he stated. Dangol said they are planning to issue a circular that those collecting such parking fees illegally will be penalised.
Earlier, the Kathmandu and Lalitpur metropolitan offices had fixed the parking fees in certain designated areas and the set fee is Rs 25 for a two-wheeler and Rs 80 for a four-wheeler for one hour.
Gogan Bahadur Hamal, director general at the Department of Transport Management, said the parking areas and the fees need to be managed by the concerned agencies. “We have not heard of such cases but if they are true then the government needs to take immediate action to stop the malpractice.”
Meanwhile, Hamal added that since the federal government had already transferred the entire authority of transport offices to the provincial governments, it is the duty of the local level governments to manage such things in a legal manner.

But the big question is, what is everybody reading? Well, for starters, everybody is reading something on their phones. Infants and adults, all are glued to their phones once they have some time off.
Some read off their iPad, some laptop while some kindle. Nevertheless, we see people always reading something these days.
Children in the past used to love being with friends and playing outdoors when they got home. They used to be creative and invent their own games, too. They loved physical activity, and parents had a hard time calling them back to study and do their homework. Sadly, it is not so these days, we hardly see any children playing outside.
They are locked in their rooms, eyes glued to some cartoons or video games. Parents still have a hard time making their children focus on their studies. I guess parents always have a hard time with children no matter what.
Nepal is a third world country, and people are slowly being led towards education. This has somewhat created hope that through education there will be change.
However, this developing nation is facing some striking challenges regarding modern technology. With almost all of its population being glued to their phones and electronic devices, rarely for productive work but rather to look at the status of people, to comment or to create memes, a lot of people are wasting their time.
Social media has created a site for gossip. This has not spared the intellectuals of our country, too. Twitter has become a site for intellectuals and high profile people to criticise and pull each other’s legs. They too are constantly on their phones, either updating other people or reading other people’s ideas. Hence, the other remaining people are left reading the criticisms and comments. None of which will lead them to actual progress in their lives.
People seem to be reading all the time. Once when I went to a shop to do some shopping, the shopkeeper was reading, too! She thought the information on her phone was more important than the person who had come to her shop. She asked what I wanted. I too, being a citizen of the reading generation, read her face and said, ‘Nothing, just looking for things’ and went out.

Leader of the Communist Party of China visiting Nepal Song Tao today called on President Bidhya Devi Bhandari and Nepal Communist Party Co-chairperson Pushpa Kamal Dahal.
During his meeting with Bhandari at the latter’s official residence, Song said China was always in favour of Nepal’s development.
Song, who heads the CPC’s International Liaison Department, said to achieve development, political parties in Nepal should enhance their capacity and be strong, according to the president’s advisor Sushil Pyakurel. Song said the MoU between the CPC and the NCP was in line with the idea of strengthening the NCP.
In her reply, Bhandari said Nepal was not a people’s republic, but federal democratic republic, and that any party could come to power through the election process.
That’s why, she said, the CPC should have harmonious relations with all Nepali parties because Nepal-China relations were above party-to-party relations, according to Pyakurel.
The meeting between Dahal and Song at a hotel in Kathmandu mostly dwelt on activities of the communist parties of the two countries.
Dahal thanked the CPC for extending cooperation to Nepal and Nepali people. He stressed the need for further strengthening relation between the two parties through continuous experience sharing. “I expect additional support from the Chinese government and the CPC in Nepal’s development endeavours,” Dahal’s secretariat quoted him as saying, during the metering.
Song stressed on the need for building a common platform for experience sharing between the two parties. Stating that ideology was one of the biggest assets of a communist party, Song said both the parties should work in further strengthening their ideologies.
“Parties can win people’s heart only through development,” said Song was quoted as saying by Dahal’s secretariat during the meeting.
Yesterday, Song had called on Prime Minister KP Sharma Oli at the latter’s official residence, in Baluwatar. According to Oli’s Press Advisor Kundan Aryal, Song said his Nepal visit was also part of preparations for upcoming Nepal visit of Chinese President Xi Jinping. Prime Minister Oli said he hoped Xi’s Nepal visit would be fruitful. Song is in Nepal to attend the two-day ‘Communist Party of China’s Opinion about Xi Jinping Thought and Ideological Discussion between Nepal Communist Party and Communist Party of China’ that kicked off here yesterday.

UNITED NATIONS: Swedish teenage climate activist Greta Thunberg shot back at on Tuesday at US President Donald Trump’s attempt to mock her on Twitter by changing her profile on the social media site to reflect Trump’s taunting remark.
Late on Monday Trump retweeted a clip of the 16-year-old’s speech to a United Nations climate summit in which she angrily denounced world leaders for failing to tackle climate change by demanding: “How dare you?”
Trump, 73, commented: “She seems like a very happy young girl looking forward to a bright and wonderful future. So nice to see!”
By Tuesday Thunberg shot back, changing her Twitter biography to: “A very happy young girl looking forward to a bright and wonderful future.”

Thunberg started missing school on Fridays a year ago to protest for climate action outside the Swedish parliament, inspiring millions of children and sparking a global climate strike movement known as Fridays for Future.

“I think I’m going to get a Nobel Prize for a lot of things, if they gave it out fairly, which they don’t,” Trump said.

Nepal Communist Party (NCP) and Communist Party of China today established fraternal relations with the signing of a memorandum of understanding here today.
The MoU was signed on the sidelines of the ongoing two-day ‘Communist Party of China’s Opinion about Xi Jinping Thought and Ideological Discussion between Nepal Communist Party and Communist Party of China’ that kicked off here yesterday.

The NCP’s International Department Chief Madhav Kumar Nepal and the CPC’s International Liaison Department’s Director Song Tao signed the MoU in the presence of Prime Minister KP Sharma Oli and NCP Co-chairperson Pushpa Kamal Dahal. Song is in Nepal to take part in the discussion.
As per the six-point MoU, the two parties will facilitate exchange of high-level visits between the two countries. The two parties will increase frequency of visits at the worker-level for training in the two countries. The parties will also boost cooperation in terms of experience sharing and expand exchange of visits of youths and local-level leaders.
The two parties will also work to boost relations at the people’s level—intelligentsia, civil society organisations and mediapersons — from both the countries. They will also run annual joint programmes on thoughts and ideology.
NCP International Department Deputy Chief Bishnu Rijal said the MoU was signed to renew relations between the two communist parties, especially after the unification of the then CPN-UML and CPN-MC. Before the integration, the CPC had separate relations with the CPN-UML and CPN-MC.
“It is an attempt to further consolidate the relations existing between the two countries and two parties, and take such relations to a new height,” Rijal said. “Nepal and NCP are committed to the ‘One China Policy’ and we are moving ahead as per the same commitment.”
The discussion programme, which ends today, is being jointly organised by the NCP’s School, Organisation and International departments, and the CPC’s Central Party School, Organisation Department and International Liaison Department.
Around 200 participants from the NCP and 40 from CPC are taking part in the event that mostly dwells on ‘Xi Jinping Thought on Socialism with Chinese Characteristics for a New Era’.
